Yeah, their overstapservice is pretty smooth. They cancel your old insurance for you, so you don't have to deal with CZ directly. I switched from VGZ last year and it was seamless.

It's super easy. You just sign up with them, and they take care of cancelling your old policy, as long as you do it before the end of the year deadline. I switched from Menzis a couple of years ago and had no issues at all.

It depends on your supplementary package. The basic insurance usually doesn't cover much, but if you get one of the higher 'AV' packages, like AV-Top or AV-Totaal, it generally includes fysiotherapie. Check the specifics of each package, it varies a lot.

Nah, not just for the Leiden region anymore. They started out regional but you can definitely get insurance with them if you live in Rotterdam or pretty much anywhere in the Netherlands. I'm in Utrecht myself.

Nope, they're national now. You can sign up from anywhere in the Netherlands. Don't worry about being outside their original 'region'.

When you're filling out the application, there should be a field for 'collectiviteitsnummer' or an option to select your employer/group. Just put your company's number there and it should link up.
